Evaluation forms

Can anyone share evaluations forms that I can use with the children? I am looking for a child friendly form to use at the beginning of a group intervention and again at the end.

Hi Andrea, Good to hear from you. What age are the children that you're looking to use this with? I ask because I'm aware that a number of people have developed pre & post evaluation forms designed for children in different key stages and also with parents.
